2009 IPF Volleyball Classic: Forms

The following are 2 forms, (that must be signed in order to participate) in the 2009 IPF-VBC.  Please download, fill them out, and bring with you on August 22nd.

1.  Minor Authorization Form:  If you are 18 or under, you need to get this form signed by you're parents in order to participate.

2.  Liability Form:  This form clears the organizers of any liability due to injury and/or bodily harm before/during/after the 2009 IPF-VBC.  This form must be signed by ALL PLAYERS.
